Precina revolutionizes diabetes care with Agentforce.

With an agent-first approach, Precina delivers daily, personalized diabetes support at scale.

About Precina

Precina transforms healthcare for patients with type 2 diabetes through daily personalized support, targeting lifestyle factors like diet, exercise, and habits to improve outcomes faster.

The Challenge for Precina

Precina wants to transform diabetes care and accessibility for everyone – but to scale, they first had to solve for their infrastructure.

With preventable diseases like type 2 diabetes on the rise, the need for more personalized, accessible care grows. Precina is on a mission to transform diabetes management by offering high-touch, personalized support‌ for every patient — ‌moving away from the traditional model of quarterly check-ups and infrequent treatment adjustments. And it works: In a pilot program with 50 patients in rural Louisiana, Precina’s unique approach reduced A1C levels—a key marker of blood sugar control—from 9.6% to 6.4% in just 12 weeks, a 3.2% reduction, compared to the 1% reduction typically seen annually with current methods.

“Diabetes is not an unsolvable problem,” said Josiah Bryan, CTO and Lead AI Researcher for Precina. “We want to scale fast, especially in underserved, rural, and low-income communities. We want to help the millions of people who need these services.”

To achieve their ambitious growth plans, Precina realized they needed to improve both their platform and processes. While effective on a small scale, their existing setup couldn’t handle the complexity of national expansion. For example, during telehealth appointments, doctors couldn’t access the latest lab results because patient data was fragmented across disconnected systems—lab reports, electronic medical records (EMRs), and imaging platforms—requiring manual retrieval and slowing time-sensitive treatment.

Another challenge was the arduous process of securing payer coverage—the approval from insurance companies, government programs, or other funding sources—needed to make more patients eligible for Precina’s services. As the company sought rapid growth, the manual steps involved in submitting the required documentation and securing approvals slowed patient onboarding.

Precina also knew it could ensure real-time access to relevant patient data—from lab results to prescriptions to telehealth records – by integrating systems and streamlining workflows. This would accelerate care delivery and support the company's expansion to meet growing demand.

And they realized the power of it all working together: Agentforce would combine agentic AI, automation, and data together in a whole new way to create a digital workforce without limits and supercharge their mission of saving more lives from preventable disease.

How Salesforce Helps Precina

Agentforce will allow for daily connections with every patient through agent-first support.

With Agentforce — the new agentic layer of the Salesforce Platform — Precina will revolutionize how they deliver diabetes care. By automating routine tasks and enabling 24/7 support, Agentforce frees providers to focus on high-impact interventions, while saving Precina an estimated $80,000 per year for every 5,000 patients in reduced administrative overhead.

Agentforce will allow for daily connections with every patient, transforming what were once infrequent endocrinology visits into a seamless, continuous conversation that supports consistent progress in managing A1C levels and overall diabetes care. By offering more frequent, continuous support, Agentforce helps patients stay engaged and motivated, fostering an ongoing dialogue about crucial lifestyle changes‌ in small steps — ‌like diet, exercise, and medication adherence‌ — ‌that lead to faster and more sustainable health improvements.

Instead of a patient waiting months for a clinical visit to adjust a medication dose or review a new dietary habit, Agentforce taps into Salesforce Health Cloud and Data Cloud to instantly access their medical records, lab results, and prescription history. It then uses that data — securely and in compliance with HIPAA standards — to deliver tailored suggestions, recommend incremental lifestyle changes, and even coordinate prescription refills.

Agentforce will also recommend personalized messaging for patient outreach, allowing providers to adjust care for each individual. Once these capabilities are live, Precina hopes the additional clinical support will improve overall outcomes. For example, Agentforce can alert paraprofessionals — trained support staff such as medical assistants, nursing aides, or pharmacy technicians — about a prescription refill that’s needed, prompting an automated phone call or text to a patient.

As Dr. Oberg, CEO of Precina explained, “Agentforce lets us identify the most important interventions and bring them forward to patients, both directly and through a provider, ensuring patients receive the right care at the right time to improve outcomes and enhance the patient experience.”

Agentforce will expand payer coverage with autonomous outreach.

Precina is currently active in three U.S. states and has ambitious plans to expand to all 50 — fast. However, one of the biggest barriers to growth is the time-consuming process of securing payer coverage, which is essential for making more patients eligible for Precina’s services. Connecting with the right patients requires navigating a complex web of insurance approvals, and that’s where Agentforce comes in. By automating contracting for every payer, Agentforce will speed up the payer expansion process, leading to faster patient onboarding and growth.

“When people think about healthcare, they don't immediately think about sales, prospecting, and selling. But the reality is that we have to recruit patients,” Oberg said. “And you go and recruit patients by getting referring providers, or referring hospitals, referring payer groups, insurance companies, and self-employed companies. Agentforce allows us to reach out and create awareness in the same way that a sales development rep would in any sales organization.”

With Agentforce taking over these complex, repetitive tasks, Precina's providers can dedicate more time to patient care and education. After confirming a patient’s payer coverage, Agentforce will proactively contact eligible patients within the network and inform them about Precina’s services. By accelerating the qualification process — especially in underserved rural communities — Agentforce enables Precina's team to engage with a large number of providers simultaneously, significantly increasing outreach.

Additionally, Agentforce with Health Cloud simplifies the documentation process for new patient onboarding, replacing the need for in-person submissions with automated, digital workflows. This improvement not only saves time but also reduces administrative burdens, allowing Precina to scale its operations quickly and provide personalized, effective diabetes care — particularly in communities that need it most.

Precina will train and coach clinicians faster and more cost-effectively with Agentforce.

Scaling high-quality care across regions requires a skilled, empathetic team. However, training clinicians and paraprofessionals is costly and time-consuming, with expenses driven by certifications, supervision, and complex systems. Precina recognized that traditional training models would slow their mission to deliver transformative diabetes care to underserved communities.

To address this, Precina will use Agentforce to quickly onboard, train, and continually coach practitioners and paraprofessionals as part of its growing patient care team. Agentforce will guide them through everything from onboarding to role-playing, therapeutic support calls, and clinical visits by mirroring sales coaching, focusing on key techniques like rapport building, setting clear expectations, delivering concise introductions, and quickly building trust.

Agentforce is HIPAA-ready and helps customers securely integrate and harmonize patient data from Health Cloud and Data Cloud as well Electronic Medical Records (EMRs), lab reports, and imaging center data. This ensures that practitioners always have the most up-to-date information at their fingertips to meet patients where they are. This integration will allow paraprofessionals to be up to 20x more productive in patient interactions, handling routine questions and providing non-clinical support, which in turn frees up clinicians to focus on more critical tasks.

Paraprofessionals will also follow clear, pre-set care guidelines to ensure clinical accuracy, promote optimal health outcomes, and maintain HIPAA compliance. If anyone, whether a paraprofessional or agent, deviates from Precina’s established parameters, doctors will be alerted to correct the situation. For example, if an AI agent provides overly long responses that frustrate a patient waiting for a doctor, Precina’s team can step in to resolve the issue promptly.

These guidelines help paraprofessionals quickly gather essential information, reducing unnecessary back-and-forth and speeding up the patient recruitment process. More importantly, Agentforce supports coordinators in building stronger relationships with patients by offering guidance on how to engage in each interaction. By streamlining administrative tasks and providing conversation tips, Agentforce frees up time for coordinators to focus on deeper, more personal interactions, creating a warm, supportive environment that fosters trust and empowers patients to take control of their health.

This game-changing training approach drastically reduces the costs and time to train while also improving results, accelerating Precina’s ability to scale. As Oberg explains, “AI is providing appropriate guardrails for a paraprofessional, so we can build resiliency in communities globally, rather than requiring more people to go through 3,000 hours of postgraduate training.” On the provider front, Oberg estimates Agentforce “will save us about $13,000–$25,000 in training costs per clinical provider per year and also increase clinical adherence rates with patients.”

We can trust Salesforce to take care of our data the way we take care of our patients.

CTO / Lead AI Researcher, Precina

Together with Salesforce, Precina hopes to transform more lives – one patient at a time.

Precina’s vision extends far beyond reducing hospitalizations or cutting administrative costs — it’s about helping patients everywhere reclaim their health, rebuild their confidence, and feel supported at every step of their journey. By partnering with Salesforce and leveraging the capabilities of Agentforce, Precina is transforming what’s possible in diabetes care, shifting from reactive interventions to proactive, personalized engagement.

Oberg said, “In my lifetime, I want to help a billion people improve their health and have a better life because of the work we're doing. With Agentforce, that dream of helping a billion people is a reality.”

By deploying a digital workforce that automates complex tasks and seamlessly integrates patient data, Precina can scale its impact without endlessly expanding its headcount. Instead of relying on an unattainably large team, Agentforce enables each existing provider to operate at a much higher level of efficiency and focus. “We probably never would have hired a thousand-person sales team,” Oberg notes. “But with this type of AI — much like a calculator or a computer — we can multiply our effectiveness exponentially.” As Precina continues to grow, it does so with the assurance that every patient receives compassionate, data-informed care. With Agentforce, Precina can increase outreach, accelerate processes, and improve care at scale — all while maintaining the highest level of attention to each individual patient.

The Salesforce Difference

Agent-first delivery is at the heart of Precina’s strategy for expansion and patient care. To find the best solution for their needs, Precina explored everything from DIY options to ready-made solutions from major technology providers. They ultimately chose Agentforce because it’s deeply integrated with Health Cloud, ensuring HIPAA-readiness and robust cybersecurity protections, as well as with Data Cloud, which brings critical patient data from external systems with zero-copy. This integration not only enables fast, personalized care but also maintains the highest security standards, with Salesforce’s Trust Layer ensuring the protection of sensitive patient data. Precina can confidently manage access permissions down to the individual patient, therapist, and provider level, ensuring data privacy and compliance at all times.

“If we were building this technology ourselves, we would have to assume the burden of the cybersecurity, upkeep, maintenance, and scalability of it,” says Bryan. “Salesforce invests so beautifully and so heavily in cybersecurity that we can trust Salesforce to take care of our patients’ data as well as we take care of our patients.”

With Data Cloud, Precina has real-time access to unified patient data, empowering both employees and Agentforce to deliver faster, more personalized care using EMRs, imaging, lab results, and more. And because of Data Cloud’s zero-copy method, Precina doesn’t have to replicate large datasets from the APIs populating the data lake. Their harmonized, 360-degree view of every patient allows Precina to mine actionable insights and automate workflows from everything they know about every patient. With Agentforce, Precina can deliver personalized care to thousands of patients at once — creating a workforce without limits. This allows them to scale rapidly, provide care to more patients, and improve outcomes, all without the need to hire a massive team. By leveraging automation and real-time data, they can meet growing demands while maintaining the human touch that makes their care truly personalized.
